Telehealth Consent

Telehealth uses electronic communications to share health information and allow a licensed provider to evaluate you remotely. It may be available in all 50 states where clinically appropriate and permitted by law.

Telehealth can improve convenience and access. Limitations may include incomplete information, technology disruption, privacy risks, and situations requiring an in-person examination.

Submitting an intake does not guarantee a prescription. The provider may request more information, labs, or a live consultation; decline treatment; or refer you to in-person care.

By continuing, you consent to telehealth evaluation, electronic communications, and appropriate sharing of information among providers, pharmacies, and service partners involved in care.
