When ED Medication Stops Working

A loss of response is usually a signal about the underlying system — not tolerance.

True pharmacological tolerance to PDE5 inhibitors is uncommon. Far more often, something upstream changed.

Check technique and expectations first

Timing relative to food, alcohol intake, and adequate stimulation account for a large share of apparent failures.

Then look at the vasculature and hormones

Worsening glycemic control, new blood pressure medication, rising lipids or falling testosterone all reduce response. Repeat labs answer this quickly.

Escalation options

Dose adjustment, switching molecule, adding a daily low-dose protocol, or combining with centrally-acting therapy are all provider-directed steps.

Rarely. A change in response usually reflects progression of vascular or hormonal factors that deserve evaluation.

What if nothing works?

Non-responders are referred for further vascular and neurologic evaluation. There are additional treatment paths beyond oral medication.
