Breaking a Weight Loss Plateau Without Just Raising the Dose

A stall is data. Before escalating medication, there are four variables worth auditing.

Plateaus are physiologically normal. Metabolic adaptation, lean mass loss, sleep debt and under-reported intake each blunt progress in a different way.

Audit protein and resistance training first

Lean mass drives resting energy expenditure. Patients hitting a protein floor and lifting two to three times weekly plateau later and shallower than those who don't.

Check sleep, stress and thyroid

Short sleep raises ghrelin and cortisol and reliably slows fat loss. A thyroid panel and morning cortisol are worth repeating when a stall is unexplained.

Then consider a medication change

If the inputs are clean and the stall exceeds eight weeks, a dose step or a molecule switch is reasonable — with new labs to anchor the decision.
